Loss of voice for Sherlock could be hysterical. Think of the fun John would have if Sherlock had laryngitis. Think of Sherlock having to rely on John for comforting things like hot beverages and mufflers - it's that or let John speak for him, which would be even funnier.
Being forced to hurt someone - I could do half a dozen of those! In WC it could be physical or emotional. Maybe neal is forced to betray Peter in order to save him. Maybe they go undercover and one is caught out, but the other can't be exposed, and one is forced by RBG (Random Bad Guy) to "deal with" the other.
Coma is just deliciousness. It might be interesting to explore it from the patient's POV.
Accidents could be just really cracky - like, Neal keeps being clumsy and people just start staying away.
